Incident Reporting & Patient Safety

Incident reporting is indispensable to the safe running of health care services. Most incident report forms identify the barriers that prevent adverse situations from developing into a major accident or disaster; they keep healthcare workforces alert and are reminders of possible hazards; and they help professionals to learn lessons, improve safety, and collect data on the quality of care on offer. Staff from UiH’s partner organisation KOSHISH Nepal were offered training on the importance of incident reporting and the creation of a care culture based on safety and  quality improvement.  With the input of nurses and healthcare assistants working at KOSHISH’s inpatient services, and following violence & prevention training, UiH supported the design of an incident report tool which is culturally appropriate, meaningful to staff, and regarded as an instrument which instigates positive change rather than blames staff for errors and mistakes.
